Individualized Supported Living

Individualized Supported Living is a living arrangement in which individuals share a home with one or more housemates and receive support 24 hours a day. Services are tailored to each person, and staff work alongside residents in their day-to-day lives.

Services are built around each person and focus on enhancing communication skills, increasing independence, teaching skills that reflect the normal rhythm of life, and fostering community involvement. Services may include:

• Enhancing communication skills
• Increasing independence and daily living skills
• Positive, individualized behavioral support
• Health, safety, and medication support
• Nutrition, meal planning, and cooking
• Fostering community involvement and relationships

Staff members work with each individual and their family to create a customized plan that reflects the person's needs, goals, and routines.
